RN Trainer

Certified Ohio Board of Developmental Disabilities

Training for certification includes:

* Category 1 Medication Administration Course

      Initial Training                                              

      Must be 18 years or older, Have proof of High School.      

      Diploma or GED, Have proof of clean Background 

      Check to take training.

*SAT (Stand Alone Training) included with  Category 1

* Category 2 (G and J Tube) and Category 3 (Insulin)

Active Cat. 1 Require before taking Cat 2 and Cat 3

Independent RN 

Developmental Disabilities Services include:

·        RN Delegation of Nursing Authority to actively 

        Certified Developmental Disabilities Medication

        Administrators (CMA/ DSP)

·        Clinical Training and oversight of CMAs

·        RN Clinical Assessments

·        Oversight of individual’s medications, treatment plan

·        Individual’s and family health and wellness education 

·        Care Coordination with Individual’s care providers

         including taking of and implementing provider orders. 

 Nurse Navigation Services include:

Networking within the community for resources.

Interaction with Health Care Providers to coordinate care services and meet clients stated needs and desires.

Health Insurance Providers to clarify and obtain authorization for benefits.

Health, Safety, and Clinical Education

Coordination of Care Services

Assist with Communications with providers in Clinical and Community settings.

Assist with applications and clinical documents

Education to benefits and resources available. 

Advocate on Client’s behalf. 

Transition of Care to home with care support, Assisted Living, Long Term Care services assistance.

For undue circumstance, I will allow for 1 rescheduling of that trainee, at my discretion. 

Additional fee for other than established locations, private classes:  Mileage is $0.67 per mile round trip each day.

Travel time is $7.50 per 15 minutes of round trip travel each day of training.

RN Delegation and Consultation

Accepts Medicaid DD clients under waiver for Delegation of Nursing Authority, RN Consultation, supervision of Certified Medication Admission Staff, medication oversite and coordination care with client's providers.

Nurse Navigation / Care Management /Assist with accessing Community Services

Private Pay Rate. 

 $45  for inital consultation / assessment (averages 1-2 hours)            

                                $35 1st hour, $7.50 each 15 min each         

                                encounter

                                4 hour gap between contact episode      

                               considered new encounter.

  Will consider rate assist to Medicaid RN rate

  for hardship situations.
